I now use Q&A captcha since reCaptcha is "broken"
I use phpBB 3.0.11, but this applies to other forum software as well.
I used to use reCaptcha, but noticed an increase the last several months in 'bot registrations. I poked around and found out that reCaptcha has been broken. I upgraded phpBB to the current version, which supports Q&A Captcha out-of-the box. I have had no 'bot registrations in the last few days.
Basically with Q&A Captcha you make up your own questions. They cannot be "simple" questions (that is answered with a Google Query). For example, "What is 1+3?" or "What color of the sky?" doesn't work.
Pick question(s) everyone would know (among your potential audience at least) and make sure the answer(s) aren't found by a Google query.

ZB Block and the Q&A captcha will give a PHPBB webmaster a pretty high wall for the spammers to get past.
I enabled ZB Block (already had seen a really slow spammer join rate after I enabled the Q&A captcha) and spammer joins have subsequently dropped to zero.
